Social Distancing Support Group Guidelines
All attendees must wear a face mask, covering their mouth and nose, for the duration of the meeting
All attendees must stay 6 feet apart
No sharing of any personal or food items
All personal contact is prohibited
Trash must be taken by attendees at the end of the meeting
All attendees must respect specific requests from others attending the meeting
All attendees verify that they (or any member of their household) have not had fever, cough, sore throat, shortness of breath, chest pain, body aches or diarrhea in the previous 7 days, have not been exposed to anyone positive for COVID-19 within the past 14 days, and have not traveled to a state outside New England in the past 14 days.
Waiver of Liability
This agreement releases The SAFE Coalition from all liability relating to injuries or illness that may occur during the time of any support group program. By signing this agreement, I agree to hold SAFE Coalition entirely free from any liability, including financial responsibility for injuries or illness incurred, regardless of whether injuries or illness are caused by negligence.
I also acknowledge the risks involved in attending a support group with other community members. These include but are not limited to illness related to COVID-19, illness as diagnosed by COVID-19, exhaustion, and/or injury while attending the meeting. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries or illness while engaging in this activity.
By submitting my electronic signature below, I forfeit all right to bring a suit against The SAFE Coalition for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
I fully understand and agree to the above terms and verify that I meet the above criteria for participation listed as guideline 8 above.
