These appointments are for adults looking for help navigating substance use and mental health care. A Collaborative Care team member will meet with you to understand what's going on, then walk with you from there—finding treatment, understanding your insurance and benefits, signing up for benefits, or connecting to resources after incarceration.
What to expect: Everyone who works with Collaborative Care builds a 90-day support plan with our team. Together we'll identify your goals and needs, connect you to the right longer-term resources, and set up regular check-ins to help you stay on track. Maintaining the plan with your team member is part of receiving support through Collaborative Care.
